You'd be taking more from the lake itself if you didn't have the perk. Soooo if the "lake" is practically an underwater landfill, you're still taking in rads from the other ♥♥♥♥ in there. For example, if you fight a ghoul while swimming and take a few hits, you'll still get the rads from the ghoul. I'm going to assume the perk makes ONLY THE RADS FROM SWIMMING ITSELF go away, not rads from anything else. Doesn't matter if it's a lake, or off the coast. Therefore if it's not a bug, then here's my guess: Water in general, unless in specific areas where it's being treated, gives you rads. And also I found this post after googling it, because the same thing happened to me. Well the thing is, the perk flat out says you're immune to taking rads while swimming.
